Preparing for Your First Visit

Important items to bring with you to your first appointment include: 

• Medication list
• Notes from previous doctors’ histories and examinations
• Copies of lab tests/reports
• Films or digital images (on CD) of brain imaging

If you do not have copies, please ensure the referring provider has send these to the clinic ahead of the visit.
