Patient Conditions


To check a patient’s condition, please call our main directory 508-334-1000 and provide the full name of the individual.  If the patient is in the directory, you will be transferred to the unit where the patient’s condition will be described according to the American Hospital Association-approved Patient Condition Descriptions

Patient specific information may not be released to the media unless the inquiry specifically indentifies the patient by name.  Information can then be provided on a patient’s general condition using the labels below.  Location – inpatient, outpatient or emergency department – may also be provided under the same condition.

  • Undetermined - patient awaiting physician and/or assessment
  • Good – vital signs are stable and within normal limits. Patient is conscious and comfortable. Indicators are excellent.
  • Fair – vital signs are stable and within normal limits. Patient is conscious but may be uncomfortable. Indicators are favorable.
  • Serious – vital signs may be unstable and not within normal limits. Patient is acutely ill. Indicators are questionable.
  • Critical – vital signs are unstable and not within normal limits. Patient may be unconscious. Indicators are unfavorable.

Call patient information – 4-6475, give name to representative.

Rep will give location of patient. Ask to be transferred to the nursing station of that unit.

If patient has died:

  • Ask if next of kin have been notified.
  • It is acceptable to report that patient has died, and the time and date of death.

All patients have the option to “opt out” of our patient registry, in which case their name will not be listed, and won’t be found by patient representatives.  This is a typical scenario in instances of crime.  We may simply report that “we have no patient by that name listed in our registry.”
