Mini Grant Funding Opportunity

The UMass Memorial Medical Center Community Benefits Department is committed to the continued efforts of our partners and UMass Memorial Medical Center caregivers to improve the health outcomes of our community members, especially those most adversely affected due to historical and systemic oppression. Dialysis|AV Access Program

Expert vascular surgeons provide 24-hour care for patients in need of emergent, short-term and long-term dialysis access. Through a close partnership with the nephrologist and dialysis center, we create a comprehensive, individualized dialysis access plan that prioritize the needs of each patient. 

Our Dialysis Access Program is ready to assist patients at any phase of treatment, including assessment, revision and maintenance.

Patient Partner Program

Benefits of Our Patient Partner Program

Our Patient Partner Program participants benefit from the insight of a peer mentor who is trained in topics such as listening, problem solving and confidentiality. These volunteers offer emotional support, practical coping tips and more. They guide patients to UMass Memorial Health programs and community resources. They also help relieve fears and concerns because they understand what it's like to face cancer. This program is managed by our social work team and designed in collaboration with our Patient and Family Advisory Council.
