Hospital at Home

UMass Memorial Health’s Hospital at Home program delivers high-quality, hospital-level care in the comfort of your home. It’s for people recovering from short-term (acute) illnesses, like skin infections, the flu, worsening heart failure or kidney problems.

Specially trained clinicians visit you several times a day, supported by telehealth technology that keeps you connected to your UMass Memorial care team. This model can help you feel better faster and stay more comfortable during your post-hospital recovery.

Barbara’s Hospital at Home Experience

After a foot injury sent Barbara to the hospital, she was eager to return home. Hospital at Home enabled her to do just that, with nurses visiting twice a day and a care team checking in remotely. Being at home, in her own bed and surroundings, made a big difference in her post-hospital recovery.

How Hospital at Home Works

We deliver the same services you would receive in our hospital. In addition to regular in-person and virtual check-ins, additional help is available 24/7.

Your personalized hospital at home care plan may include: 

• Bedside diagnostic testing, such as blood draws and basic imaging studies
• Medical supplies
• Medications
• Rehabilitative therapies
• Remote monitoring of vital signs, such as temperature, heart rate and blood pressure
• Respiratory care, including oxygen
• Support from social workers and care coordinators

Our Team Approach

When you receive care at home, you’re never on your own. A dedicated team works together to support your post-hospital recovery. We also communicate with your primary care provider and any UMass Memorial specialists you are seeing.  

The Hospital at Home team includes:

• A doctor or advanced practice provider who oversees your care and checks in with you daily
• Registered nurses or paramedics who visit you twice a day or more to perform additional assessments or care as needed
• Rehabilitation providers, such as occupational, physical and speech therapists, who visit as needed based on your treatment plan

What to Expect

If the Hospital at Home program is right for you, a member of our team will meet you at home shortly after transfer. We help you get settled, including working with you to determine the best timing for daily visits from your care team.

While participating in Hospital at Home, you are connected to your care team 24/7. We use devices that allow for remote vital sign monitoring and telemedicine visits.

Before you are discharged from Hospital at Home, the care team works with you to coordinate any additional services you may need.

You can also reach a member of our team at any time. If your situation changes, specially trained nurses and paramedics perform an in-person assessment and bring you back to the hospital if needed.
