Care at Home for Complex or Serious Health Needs

When you're healing from a hospital stay or living with a serious illness, there's comfort in being home. Our programs bring the right level of care to you, so you can rest, recover and focus on what matters most. Offerings include:

Hospital at Home

Eligible patients receive hospital-level care at home for certain conditions, such as severe dehydration, the flu, infections and complications from heart failure and lung disease. Postpartum Hospital at Home

Digital Health Clinic

This service gives your care team a clearer view of how you're doing day to day. Easy-to-use technologies track key health signs like blood pressure or weight and send the information to your providers. This helps them make timely changes to your care and offer additional support if needed. 

Mobile Integrated Health

Specially trained paramedics visit you at home and connect with a hospital physician through telehealth technologies. They assess symptoms, perform basic tests, and deliver treatments for issues such as infections, dehydration or breathing problems.
