Criteria (revised as of 2022) is Based on:

• HealthAlliance-Clinton Hospital’s most current Community Health Needs Assessment
• 2022-2024 Community Benefits Strategic Implementation Plan

Tier 1 Funding Details:

• Grants of $10,000 or less.
• Rolling application reviewed/awarded quarterly.
• Completion of online application including budget detail required.
• Fund priorities based on new priority areas and target populations.
• Cannot be for event sponsorship.
• One-time/per agency/annually.
• Does not preclude organization/agency for applying for Level 2 and Level 3 funding.
• Will consider how application is to support capacity building; and diversity, equity and inclusion initiatives if project advances efforts to address priority areas.
• Capital improvement request will require landlord approval (if applicable).
• Fiscal agent needs to be clearly identified and letter of support provided (if applicable).
• Upon receipt of the grant award, funds need to be committed within 3 months of award and spent down within 12 months.
• Additional financial information may be required before grant funds will be released.  A W-9 will be required.

Public announcement – Request for proposals opening date: Monday, February 12, 2024
Tier 1 online application deadline: Friday, March 8, 2024
Tier 1 awards announced: by Friday, March 29, 2024

Round 2 (grant awards totaling $50,0000):

Public announcement – Request for proposals opening date: Tuesday, June 11, 2024
Tier 1 online application deadline: Friday, July 12, 2024
Tier 1 awards announced: by Friday, August 2, 2024

Tier 1 Reporting Deadlines:

Round 1:

Six-month reporting deadline: 9/30/24

Annual report deadline:  4/28/25

Round 2:

Six-month reporting deadline: 1/31/25

Annual report deadline:  8/1/25
