If you are living with a chronic condition, like hypertension or type 2 diabetes, remote patient monitoring and other digital home treatments offer extra support between doctor visits. These services enable us to spot small changes early, so we can intervene to help keep your care on track.

Remote patient monitoring and other digital health services are part of our connected, systemwide approach to care. UMass Memorial Health is one of the few programs in the region offering this level of coordination. By staying one step ahead, our team helps you feel your best and lowers your risk of complications.

How Our Remote Chronic Disease Management Services Work

Using connected devices, we track important health signs  — like blood pressure, weight or blood sugar — and securely transmit the information to care teams. This gives us a clearer picture of how you're doing between in-person visits.

If you aren’t feeling well or we detect an unexpected change in your health signs, we reach out to you right away. We may:

• Call you to better understand what may be affecting your health
• Contact your provider to discuss possible updates to your care plan, like changing a medication dose
• Schedule an appointment with your provider if needed
• Send a paramedic to check on you at home through our Mobile Integrated Health program

Congestive Heart Failure (CHF) Care

Heart failure can be unpredictable, so we pay close attention to small shifts in your weight, blood pressure and other key health signs. By spotting changes early, we can alert your provider and help you stay well at home.

Virtual Pulmonary Care

For chronic lung conditions, like chronic obstructive pulmonary disease (COPD), our virtual pulmonary rehab program helps you breathe easier and stay well. A respiratory therapist guides you through personalized exercises and checks in regularly to monitor your progress.

How Much Does Remote Patient Monitoring Cost?

Most patients have little or no out-of-pocket cost for remote patient monitoring. We check your insurance and let you know if there’s anything you might need to pay. In many cases, the service is fully covered. If there is a cost, it’s typically a low copay.

How Do I Get Started With Chronic Condition Care at Home?

If you're interested in remote patient monitoring, talk to your primary care provider or specialist. They can place a referral to the Digital Health Clinic, which will take care of the rest. Once we receive the referral, a member of our team contacts you to explain how the program works, answer your questions and make sure you’re set up with the right equipment.

Remote patient monitoring has been offered at UMass Memorial since 2023 and now supports hundreds of patients.

Any specialty can make a referral. The Digital Health Clinic handles all setup and logistics, including patient outreach, equipment coordination and ongoing monitoring. Our team stays in close communication with you and your patient to support ongoing care and respond quickly to any concerns.
