The UMass Memorial Medical Center is a participant in the new Transforming Episode Accountability Model (TEAM) which launches January 1, 2026. TEAM is the latest CMS Innovation Center initiative focused on improving quality of patient care and controlling costs through episode-based payment models.

Policy for Selecting TEAM Collaborators
Purpose
To establish a uniform, compliant process for identifying, vetting, and approving individuals and entities to serve as TEAM Collaborators under the CMS Transforming Episode Accountability Model (TEAM), consistent with 42 C.F.R. Part 512 Subpart E and applicable UMass Memorial Health standards for conflicts of interest, compliance, and quality performance.
Policy Overview
Any potential TEAM Collaborator must:
- provide evidence of a demonstrated commitment to the principles of Value-Based Care and quality improvement; and,
- be enrolled in Medicare or employ practitioners enrolled in Medicare.
Selection criteria are not based directly or indirectly on the volume or value of past or anticipated referrals or business otherwise generated by, between or among the TEAM participant, any TEAM collaborator, any collaboration agent, any downstream collaboration agent, or any individual or entity affiliated with a TEAM participant, TEAM collaborator, collaboration agent, or downstream collaboration agent.
Definitions
TEAM: Transforming Episode Accountability Model, administered by CMS
TEAM Participant: An acute care hospital participating in the TEAM.
TEAM Collaborator: A Medicare enrolled individual or entity that enters into a sharing arrangement with a TEAM Participant
